What is transition?Transition service requirements in IDEA are to improve the quality of life of young adults with disabilities. The basic purpose of the legislation is to better prepare students with disabilities to enter the workplace, go on for further training, become as independent as possible, and contribute to society. Further, the intent is to enable students to gain access to the services that are necessary for them to accomplish the outcomes they desire and to have services in place before they leave school. The legislation speaks to focusing the IEP development, planning, educational programs, services, and supports toward post-school outcomes. This process should promote successful movement from school to post-secondary education, training, employment, independent living, and community participation. Transition and the Wisconsin Statewide InitiativeCESA 10 is involved in the Wisconsin Statewide Transition Initiative, which is based on a comprehensive approach to providing transition services in the state of Wisconsin. The Wisconsin Statewide Transition Initiative website is located at www.wsti.org. Transition Action ModelThe Transition Action Model depicts activity that the CESA 10 Transition Action Teams and the Transition Action Councils have been working on, including the Point of Entry Manual (POEM), Interagency Agreements, District Transition Plans, and other items. The model is designed to help all interested teachers in locating information that could assist them in better serving their students during transition planning.
